Athletic forms are required each year your son/daughter participates in athletics at Cornerstone Christian Schools.

An updated physical exam and medical history is required for ALL student athletes PRIOR to the start of the sports first practice for the school year. These forms are good for one year from the date received.

No student will be allowed to compete in a sport until their MEDICAL HISTORY AND PHYSICAL EXAM FORM has been turned into the Athletic Department AND the Electronic Athletic Participation Forms have been submitted online and approved by the Athletic Department.
